Partner agencies of the DAAT are asked to complete the below monitoring form when receiving requests from agencies/services to deliver drug and or alcohol awareness/information events to parents and carers.

The information obtained from this form will be used by the DAAT partnership to gain, both a better understanding of the number of parents and carers who are receiving drug and alcohol information and also to investigate possibilities of duplication of requests for agency/service input.
